When you file Form I-485, you must also submit Form I-693, Report of Immigration Medical Examination and Vaccination Record. A civil surgeon is responsible for providing you with a completed Form I-693 that is signed and placed in a sealed envelope.
- Only a USCIS-designated civil surgeon can complete and sign Form I-693.
- Regular family doctors cannot sign this official immigration form unless they have this special government authorization.

Government-issued photo ID, such as a valid passport or driver's license.
- Form I-693, with Part 1 filled out in advance (do not sign it until the doctor tells you to).
- Official vaccination records. List of current medications with doses.

정부24 예방접종증명서, 질병관리청(KDCA) 기록, 병원 또는 보건소에서 발급한 예방접종 기록, 아기수첩(예방접종 날짜가 기재된 경우), 해외 의료기관에서 발급한 공식 예방접종 기록 등 가지고 계신 예방접종 기록을 모두 가져와 주세요.
3하루 만에 Form 3231을 받을 수 있나요?
대부분의 경우 가능합니다. 다만 필요한 예방접종이 있거나 추가 확인이 필요한 경우에는 일정이 달라질 수 있습니다.
